Unless the guild/alliance/just on the same realm are running a few duos separately. TW does that sometimes. People will queue together quasi pug when nothing is happening or other groups are full..

It should be single queue only. Dota tried duo queue but its too much an advantage. That way it's a greater chance randoms will be added instead of split premades.
