It's only with the Bloodlord set ?

That's the only set it appears to be on according to the current armor guide pics. The only other possible places you might find it would be the rare fortune set or from Lair bosses that drop a helm item, like the Bandit Queen in T1. If I remember correctly one of the T3 epic quests drops a helm too, but it has been a while since I did any of those. The problem is I don't know if any of those actually drop that helm appearance, just that they tend to have rarer appearances in general. Hopefully another orc will read this and have more helpful info!
