Witch of the North bugged?

Hello,
I'm wondering if there is an issue with the Witch of the North event. For some reason I cannot get credit for completing the PQ even though I've killed her 4 times while in a warband.
Also, does anyone know where the Firehawk of Dahz pops up at... I've been looking everywhere in Praaq and can't seem to find it.
