The number of kills / medallions is changeable. Not something I see as fixed .
I am very sure that kill per medallion number can be arrived at to give the desired progression rate.

Yes there is a issue around which poison to take. The current killing blow or the old reward model of kill quests. Both have different side effects. killing blow favors 1v1 and dps centric small groups while kill quests is a bit more generic with tapping targets and Hotting dps being "good"...
