Playing for a bit at my folks place in the country this issue is stark. I've noticed it now for awhile even in the city, and it makes playing a non-aoe melee character even more clunky.
Simply put very often my target is either out of the los cone, or out of range. That is fine, however my melee abilities fire, go on CD, and do nothing. It is extremely frustrating to say the least, particularly in regards to KD and punt.
The server should not be putting abilities that don't fire on Cooldown. Melee los and range is currently broken imo.

If you're playing with ping, the new LOS/range checks for abilities are extremely punishing. Because the ping is so bad, the game's client-side interpolation makes it so players walking near walls, appear to walk into them, breaking LOS on any ability that has a build time.
