It should do damage every time i attack not only the first time i, moreover i couldn't try it in group so i cannot fully answer you.
Every memeber of party for 5 sec should do 50 spi damage every time they hit too, but considering it was bugged on official, i give you another 2 try to fix it :P

No they dont need to get toned down. They still do small damge. Before they were doing close to none.
I hope that devs wont listen to such opinions. On officials u couldnt solo aisha and mobs were pretty touch.
Guys please dont force easy-mode on devs !

uh no it shouldn't

"For the next 5 seconds, Tooth Of Tzeentch will cause an additional 150 damage on the next damaging attack made by everyone in your group."

Not "For the next 5 seconds, Tooth of Tzeentch will now cause an additional x damage on any attack made by anyone in your group."

But if you want maybe it can be set to deal shitty damage every time someone crits like on Swordmaster...

Just fixed a bug to do with quest objectives which allowed a player to never die :O
