It's a good question actually, I can't tell if we have 100% of all weapons in here (I doubt it actually), but all new released don't have unique skins, like City Dungeons weapons have the appearence of former T4 epic ones.
I dunno about Reverence or Bloodlord, but at least Norkalli knows if there are new unique ones at all.

We could add hints like e.g. old Carnage / Red Carnage = new Bloodlust 2h hammer.

Pretty much this.
Most new weapons don't have new Skins.
So if one Skin has one Item you can get in the game I'm happy with it.

61rrr SC 1h axe (defensive one) has new unique look, i can post it some time later
Also one of 61rr SC 2h axes seems to have unique look similar to 45rr 1h axes, but i dont have it yet

Also some of the weapons (likw 2h axes #11, #13 and #14) are still undiscovered

I think i saw 2h axe #10, i'll check it out and post the name here, i think its available as low lvl influence reward somewhere.

- Doomstriker Greataxe - Dwarf Chapter 19, Dragonslayer Ridge in Thunder Mountain
- unconfirmed Mititempered Waraxe - Dwarf Chapter 17, Lorkinson's Excavation in Thunder Mountain
2h axe #15 (subjugator one) is also available from some influence rewards:
- Deathwalker's Trollsplitter - Empire Chapter 22, Fires of Sigmar in Chaos Wastes
- unconfirmed Trollsplitter of Herron's Rage - Dwarf Chapter 21, Oathgrund's Watch in Black Crag
- unconfirmed (Ironbreaker) Mititempered Greataxe - Dwarf Chapter 17, Lorkinson's Excavation in Thunder Mountain

Looks like influence rewards can be different for IB and Slayer, so need someone to check these out (at least by comparing icons)

#10 and #11 are supposeduly quest rewards in T3, but i cant check, did them long ago.

Look #14 is still undiscovered for me, although it might be one of 61rr axes
