Try collecting a bit more info for us mate.
https://www.google.com/?ion=1&espv=2#q= ... dump+files
Enable dump files in windows following these instructions.
Next download this program.
http://www.resplendence.com/whocrashed
It's nothing crazy, it will just interpret your dumpfiles for you, and should point you towards any problem drivers that may cause the crash.

My personal suspicion is that it's faulty drivers, or bad ram/GPU. I would try memtest86 first just to rule out the ram and the dimm slots on the MOBO. Then analyzing dumpfiles with whocrashed to see if there's a broken driver related to the crash.

Here is the error code itself defined by microsoft.

Alright here is the dump analyzed by Whocrashed:

On Sun 6/26/2016 5:00:09 AM GMT your computer crashed
crash dump file: C:\Windows\memory.dmp
This was probably caused by the following module: Unknown (0xFFFFFA8009E534E0)
Bugcheck code: 0x116 (0xFFFFFA8009E534E0, 0xFFFFF8800495E3C4, 0x0, 0xD)
Error: VIDEO_TDR_ERROR
Bug check description: This indicates that an attempt to reset the display driver and recover from a timeout failed.
A third party driver was identified as the probable root cause of this system error.
Google query: VIDEO_TDR_ERROR

Of course, my luck, the culprit is 'unkown', can you make anything out of this?

EDIT:

I love it, now my computer can't crash, instead it just acts like it will 4 or 5 times and then just my GPU shits out, and everything else is going on in the background. I can hear the sounds, the game reacting to my key strokes, but just a black screen no matter what until I reset my PC.

It really seems like its a driver issue, but what can I do about that? I have the most updated Nvidia drivers for my card?

I can't make anything of it other then what bugcheck code itself.

According to that it could be drivers, memory, mobo, hardware configuration, faulty/inadequate power supply, overheating.

I would go down the list checking stuff off one at a time.

Great little program just to check temps quickly, useful even to see idle temps just running the OS. Running WAR my gaming PC sits at 38*C on the GPU and 29*C on the CPU just as a comparison, lower when idling in windows.

We come back to the memory test, and again I recommend getting a USB flash drive, download memtest86 usb flash booter and run it, select the USB drive when prompted by installer. Then reboot the machine and go into the BIOS, set boot priority to the USB flash drive and let memtest86 do its thing for 8 passes to pretty much confirm good memory/dimm slots, it will take awhile. Errors typically show up very quickly, within the first couple passes usually.

I'm not sure how you went about installing your graphic drivers, but it wouldn't hurt to redo it. You could also try a bare minimum driver installation using a method like this:

I am idling now at about 38c for my GPU and 33c for my CPU. My buddy and I just took apart my computer and GPU and dusted everything out and repasted. Unfortunately that didn't solve any issues, Instead of going into failsafe fan mode because my GPU is getting too hot, the driver just up and fails.

I have a very shitty stock fan on my CPU, but that doesn't seem to be what the problem is. I haven't had a chance to run memtest, I will tonight. I am just confused why GTA V, Planetside 2, TW:Warhammer, Warframe and Fallout 4 play perfectly fine at high graphics, and this game crashes my GPU instantly. That again makes me think the issues isn't really on my end, or if it is, its not something I can fix easily (IE: The game not being compatible with certain GPUs).

You can try to download the nvidia inspector.
Set the maximum FPS for war.exe to 30 (or lower/higher). Create a profile for war.exe if there is none.

Are those settings what I should be aiming for in the spoiler? or just an example of what everything looks like?

Frame Rate Limiter is the setting for the maximum frames per second, the rest is for somthing else. As i understand it, your card is overheating and i think with restricting the maximum FPS the problem could be gone. Maybe the WAR client lacks a max FPS setting and allows theoretically unlimited FPS.

I appreciate the help man, but now the launcher is refusing the connect. I click connect and nothing happens.

EDIT: Rebuilt the performance counters, still not connecting. I'm going to try redownloading the launcher.

Buy a new video card they are cheap now that the 1070 and 1080 are out, something in the 780 or 900 series there are some nice 600's also out there, better than the 560 ti. check craiglist, ebay, or offerup and letgo phone app. if i was having this much trouble with that card and it drivers i would sell it and buy something else.

There are several issues with your statement. I am not having trouble with my card. My card is handling new AAA titles without a hitch. The issue is THIS program. If I had money to go buy an new graphics card, I'd drop 200 bucks on a 4gb 960.
