Need help with UI setup!

Hello, I am trying to get a UI like this one: viewtopic.php?t=6808 but the link provided to the google doc requires access to the link. Does anyone have a similar UI I could use?

That UI setup wasnt updated for ages, I`d go with Viny UI, which look similar.
