Pretty much it. Also, I don't know if it is more of an NA thing but the T3 cap and the "in development" state has led to more of a "wait and see" feeling with the people I know. Unfortunately at the moment, a bigger issue than incentivizing defenders is incentivizing attackers to even attack. You can't defend a keep after all, if it is never attacked.
At the moment, attacking isn't occurring either.
I think one of the best ideas i've seen so far is Tesq's idea to add a keep take requirement to the new Gifts of War/Gears of War quest.
thus, inaddition to the 150 kills..2 keeps have to be taken as well. this makes rvr required. I love this idea.
I think this could get the orvr rolling.

Hamfisting 2 keeps into the quest will just be an annoyance, and still wont incentivize defending keeps.
People need to feel like they are progressing constantly or they will get bored. RR cap needs to be increased, even if as someone suggested at a gimped rate after you hit 40.
While some people can play this game without any progression and be completely content, I really think the vast majority of us do need that carrot on a stick. And in this game you need everyone participated in ORvR for the gameplay outside of scenarios to be fun and exciting. Taking empty keeps, or defending with so low numbers you are lucky to score even 1 kill is not fun for either side.
People will hit a point where the only way to get good renown is to chase AAO, meaning that people will be defending. And the attackers will in turn have the incentive to care because now they are actually getting rewards for taking keeps that are being defended. Not to mention a lot more strategy will have to go into successfully capturing a zone due to 6 mans and other types of gank groups prowling around BO's constantly.

Forcing people to take a keep to earn medallions from the quest is only going to exacerbate the problem Desto currently has in NA times. Destro does not skip ORVR because they are not incentivized, they skip it because Order dominates T3, as they did T2. Right now you can run SCs and fight with even numbers to earn medallions from kills and the quest, I don't think we need to put ourselves on the tracks to a gear gap.

They dominate t3 because destro has no incentive to defend keeps with RR40 characters, spamming scenarios is just way more lucrative in terms of medallion farming if you have an organized group at that level.
the power gap wouldn't even be that major. 34 renown ranks is equal to ~120 of 1 stat extra. The amount of sheer grind to go from RR 40 to RR 74 would be staggering anyways. There would be very few people getting over that hill. Not to mention there are MUCH bigger power gap problem right now with the level 22-31 SC bracket which should be level 31 only. And devastator gear sets are going to play a more major role in character power than renown ranks will.
