You can get a helm with that same appearance from Shambler in T3 or Metoh in T4. Check the Lair section in Wiki for more info.

If you want to get the Ekrund lair one (Casque of the Bandit Queen), then you'll have to either debolster and get it yourself or buy it on the AH. All lair items are BoE again. Knowing Order AH, prepare to dish out 50+ gold for that.
However, I am 95% certain that one of the T4 PvE influence rewards actually has the same appearance.


Edit: Actually I just looked it up. I think the reward for chapter 22 Elf (Casque of the Banisher) has that appearance. I don't have that item anymore, so no guarantees I'm afraid.
Also, currently the Casque of the Bandit Queen is priced at 100g- 175g. Seems I was a tad optimistic with my estimate.

Ekrund lair (t1) and Metoh lair (t4 in Troll Country) have exact same looks
you can try doing it in either place, get lucky and win first time, or attempt a dozen kills and never be lucky and then just end up buying it from AH for something like 50-100g because you don't want your kotb to look stupid.

As usual, personal taste varies. For my knight i still use the one from the T3 epic dwarf quest chain, casque of the ancient. Only one bloody feather, front visor only hides half of the face, with some colours it looks good... but as i said, others will have different preferences.

On a more serious note, my problem with KotbS headpieces is largely that you are severely limited in your options if you want a closed helmet. Apart from the Bandit Queen casque, they're pretty much all variations of open ones. There are others, but those are apparently not currently implemented in any form, and while I actually like the Bandit Queen helmet well enough, I have been wearing it since level 10. A bit of variety would be nice.

Unfortunately, none of the other T4/T3 helmets really do anything for me.

Bonus points if you textmacro "You're coming with me, dead or alive!" on your Shackle.

There is another helmet that looks similar to the Ancient One, but with a sun ornament instead of the feather. Might be conq or dom, not sure.

Let me hijack this thread for a moment: If anyone knows a nice looking greatsword, I'd be grateful for some pointers. Dawn is cool, but everyone uses it, which, together with the Bandit Queen helmet, makes all the 40 2H KotbS look pretty much the same. So far I have not had much success with finding more unusual greatswords. The inf rewards all look quite plain, I don't particularly like the look of the R39 scen weapons, and everything higher up is for the foreseeable future out of my reach. Anyone have a picture of the Gutslime weapon maybe?
