Less of a suggestion, and more of "put it in the air, see what will came out of it".
Age of Conan mmo had a server called Rage with an interesting rule set:
1. Open pvp everywhere, and no guards anywhere.
2. Everyone can attack everyone, no matter lvl (and there wasn't any kind of boost)- and if I remember right, you could attack even your guild members. Obviously, there were no realms.
3. Player loot- if you died, several items + gold would drop from you, and could be looted by the killer.
4. All classes could use stealth- and using food for regen, could stay pretty much permanently in it.
Now in the end of the day War rvr is much better, but that rule set did made for quite an interesting gameplay. Now I'm not sure what exactly can be done with it, so its more of a "food for thought".

FFA pvp would be fun for a "Death Night" style event. Khaine, Lord of Murder, etc. Cant see it ever becoming a permanent state of things though. Even if the coding required is feasible, it runs counter to the spirit and style of the game.

Warhammer also started with a few open PVP servers. Unfortunately, it didn't work in AoC and it didn't work in WAR -- Those were some of the first servers shut down, in both cases. This would require a new layer of RvR rules, would probably be restricted to a specific zone (Isle of the Dead seems appropriate), and would take a whole lot of work and effort from the entire team to implement.
